Step 1.Temporary Food Establishment Permits

• You will be contacted within 2 business days from 817-392-7255 to pay by credit card once the application is submitted.

Step 2.You will need the following to fill out the form online.

• Name
• Description of proposed event or activity
• Start Event or activity
• Start time
• End date
• End time
• Street Address of the event or activity
• Name of he organization
• Contact information of responsible party
• List of foods to be served
• Places the food is prepared and stored ( only listed food and beverage are authorized.)

Step 3.Read the disclosure and confirm the statement

Temporary food/beverage stands must comply with all Fort Worth requirements, including required sinks, food temperatures, stand construction, etc. All workers handling food must have a Fort Worth Food Handler Certificate. If the above organization is a non-profit entity, there may be an exemption from this requirement by providing the tax exemption form to the Consumer Health Division.

Step 4.All temporary event workers must have a food-handler certification.
