• SummaryThe City of Fort Worth (City) invites participation in the Request for Proposal from qualified vendor to provide healthy vending machines service on as needed basis for all city departments per the attached specifications. This Agreement shall begin on MM/DD/2019 (“Effective Date”) and shall expire on MM/DD/2022 (“Expiration Date”), unless terminated earlier in accordance with this Agreement (“Initial Term”). Upon he expiration of the Initial Term, the Agreement shall renew under the same terms and conditions for two (2) one-year renewal periods, unless City or Supplier provides the other party with notice of non-renewal at least 30 days before the expiration of the Initial Term or renewal period.
