The Consolidated Plan (ConPlan) is the five-year strategic plan to identify housing and community development priorities that align and focus funding from the United States Department of Housing and Urban Development (HUD) Community Planning and Development (CPD) formula block grant programs:
- Community Development Block Grant (CDBG) Program
- HOME Investment Partnerships (HOME) Program
- Emergency Solutions Grant (ESG) Program
- Housing Opportunities for Persons with AIDS (HOPWA) Program
The plans meet federal requirements under the Community Development Act of 1974 and the National Affordable Housing Act of 1990, as amended. The plan was submitted to HUD to meet application and regulatory requirements for the block grant programs.

About the 2023-2027 Consolidated Plan
The 2023-2027 ConPlan presents a summary of housing and community development needs of low to moderate income households in Fort Worth, and identifies the priority needs that the City proposes to address with HUD grant funds over the next five years.
The ConPlan also states the City’s goals for addressing the identified priority needs. These goals must align with HUD’’s goals for providing Decent Housing, a Suitable Living Environment, and Expanded Economic Opportunities for low-income persons. Each annual Action Plan during the five-year period from October 1, 2023, to September 30, 2028, must address these priority needs and goals.

The 2023-2024 Action Plan for the City of Fort Worth summarizes the specific activities and proposed expenditures for the CDBG, HOME, ESG, and HOPWA Programs.
The purpose of these funds is to benefit low and moderate-income families, individuals and neighborhoods throughout Fort Worth. ESG grant funds benefit Fort Worth homeless persons, and HOPWA grant funds benefit individuals with HIV/AIDS in Tarrant, Johnson, Parker, and Wise counties. Activities and/or programs which receive funding must contribute to the attainment of goals outlined in the Five Year (2023-2027) Consolidated Plan.
The total amount of grant funding to be allocated for the 2023-2024 Program Year is $13,452,621.00. This includes $6,994,785.00 in CDBG funds, $3,271,119.00 in HOME funds, $610,543.00 in ESG funds, and $2,576,174.00 in HOPWA funds.
