The complaint and commendation process is vital to the promotion of police accountability and transparency.

OPOM welcomes your compliments of FWPD personnel. Once received, OPOM will track the accolades and forward them to the FWPD. The OPOM is NOT part of the Fort Worth Police Department.

 

How to submit a commendation

  • Follow the instructions after selecting the form.
  • Please enter the information into each section.
  • Once completed, press: Electronically Sign and Submit and the form will be sent electronically to the Office of the Police Oversight Monitor.
